I once played a buffing oriented bard, me and the rest of the team were mercenaries on a journey to the enemy territory. We hired a ship to get there, crossing the vast sea. The GM is a very good storyteller and he likes to describe details of our surrounding. So he describe how the waves crashing down our ship in an epic way, seagulls perched on our masts, dolphins and a giant squid passes by. Then, all of the sudden, our attention seeking barbarian threw a javelin at the squid before our GM finishes his narration. We were all like gave "dude, what the hell?" look on him. He boastfully said "That's a monster right?" The GM replied "No, giant squids are common animal in this region." Then the barbarian continues "Is it hostile?" The GM said "After you threw a javelin at it? Now, it is." We rolled our initiative and the barbarian almost got himself dragged and drown by the squid.

Well I was playing a 3.5 game and the most of the participants were missing. The only people attending were me, the GM and the group's sorceror. We go into the dungeon and we defeat the final encounter (made easier because we were only 2). The GM starts listing the loot. Amongst the loot was a flask with a reddish liquid in it. The sorceror goes: "Hey, I bet this is a healing potion." I jokingly say that he should drink it. He does just that. The flask was filled with a burning liquid. he takes 2 damage and almost dies.
We then check the room. In the middle of the room is a fountain with a clear liquid in it and marked with magic runes. At the bottom of the fountain is a sphere. I poke it with my sword. Nothing happens. The mage then decides it's a good idea to taste the liquid. He had 2 hit points left. The GM makes him roll fortitude and he saves. The liquid makes him puke.
After all these he tells me: My character doesn't have too much common sense.
